PACE Place is part of a national network of programs called the Program of All-Inclusive Care for the Elderly (PACE), offering seniors and their families the care, nutrition, rehabilitation, transportation and supportive services they need to remain healthy and living in their own home. PACE Place has been approved by the Centers for Medicare and Medicaid and the State of Florida.
Our clinic and beautiful adult day health center are located at 5450 Ramona Boulevard, Jacksonville, FL 32205. PACE Place’s #1 priority is to assist elders in maintaining their independence and providing support to remain at home. Our team understands the demands of being a caregiver and helps with coordinating all care and services including transportation. Caregivers can relax knowing PACE Place is looking out for their loved ones.
With the PACE Place, getting older does not mean losing your independence. Our participants live full and meaningful lives – and often say they have never been as active or satisfied. As care providers, it is our privilege to help our participants not only stay healthy, but also stay independent and active.
PACE Partners of Northeast Florida’s PACE (Program for All-Inclusive Care for the Elderly) program or PACE Place is a nonprofit subsidiary of Community Hospice & Palliative Care and part of the Alivia Care, Inc., family of care solutions. Care coordination for the PACE’s Partners of Northeast Florida’s program, the first of its kind in North Florida, is delivered at The PACE Place, and at the participant’s home.

What Is Pace Place?
PACE Place is a nationally recognized model of care designed for people age 55 or older who require nursing-home-level care but would rather receive it at home or in the community rather than in a nursing home. Who May Benefit From PACE Place?
The following are situations that may indicate a need for, or benefit from, enrollment in the PACE Place program:
- In need of comprehensive care coordinated by a dedicated team of care professionals
- Fragmented support services
- Recent hospitalization due to illness or injury that has resulted in decreased functional ability
- History of multiple or frequent emergency room visits, hospital admissions, or short-term nursing home stays
- Frequent falls
- Forgetting medications
- Social isolation
- Caregiver burnout
- Wants to live at home but requires support of others to do so
- Involved in an adult daycare program but frequently misses attendance due to medical frailty or inability to use or pay for transportation
What Types Of Benefits Do I Receive?
The benefit package for all PACE participants includes: Primary Care, Hospital Care, Medical Specialty Services, Prescription Drugs (including Medicare Part D drugs), Nursing Home Services, Nursing Services, Personal Care Services, Emergency Services, Home Care, Physical Therapy, Occupational Therapy, Adult Day Health Care, Recreational Therapy, Meals, Dental Care, Nutritional Counseling, Social Services, Laboratory/X-Ray, Social Work Counseling, End of Life Care and Transportation.
Who Pays For Pace Place Services?
Payment for PACE Place depends upon your eligibility for Medicare and Medicaid:
- If you are eligible for Medicare and Medicaid or eligible for Medicaid only and do not have a spend-down,* there is no cost to you for PACE covered services.
- If you are eligible for Medicare only, you will pay the Medicaid monthly rate on a private pay basis along with your Part D premium for prescription drug coverage.
- If you are not eligible for Medicare or Medicaid, you may enroll in PACE on a private pay basis, with your cost equal to the Medicare and Medicaid rates. You will also pay your Part D premium.
There is no additional cost to you for the authorized covered services in the PACE Place provider network. If you choose to access services on your own that are not covered or obtain services of a nonparticipating provider that are not authorized by PACE Place, you may be subject to payment of these services.
*Spend-down, which is also known as surplus, is the difference between your monthly income and the monthly Medicaid rate.
What If I Am Not Currently Enrolled In Medicare Or Medicaid?
If you are not currently enrolled in Medicaid, a financial assessment to help determine eligibility is completed; the potential participant will need to provide a summary of income and assets for this assessment. If the decision is made to apply for Medicaid, a list of documents needed for that process will be provided and a PACE Place representative will help with the process.
Can I Continue To See My Current Doctors?
PACE Place has contracts with specialists, hospitals and other providers throughout the community to make sure you get all the care you need when you need it. You will receive most of your care from your PACE primary care physician and PACE center staff.
Who Decides Which PACE Place Services I Need?
Your PACE Place primary care physician and team of clinicians will work with you and your caregiver to coordinate all aspects of your care and determine which services you require.
Can PACE Place Provide Services In My Home?
Yes, PACE Place can provide services in your home, including home health aides, nurses and social workers as needed.
Is A Participant Required To Attend The Day Center?
While encouraged, it is not a requirement to attend the PACE Place Center from week to week. Attendance is based on participant needs. However, regularly scheduled doctor appointments and therapy occur out of our PACE Place Center.
